  • What's the correct use / OK to stacked on top of PLA or LOA: For Spring hires or late starts they have a gap in benefits - EE is placed on a short leave for August pay period. EE has the option for direct pay or not.

    • If not direct pay, end mandatory benefits. End core benefits if not on direct pay. Use Perm event code 533 for Medical.
    • If the EE opts for direct pay, give the employee the Direct Pay Authorization Form for Dental, Vision, Life and LTD.  Give EE  the Direct Pay Information form (Off Pay Status). Keep core and mandatory  benefits active if EE is on direct pay.

  • How Long Stay on SWB:
    • For month of August and the EE is rehired for the Fall Term. Must terminate the EE if the EE doesn't return for the Fall Term. EE is offered  COBRA if core benefits ended and  they were previous on direct pay.
    • EE is not offered  COBRA if core benefits ended and  they were not previously on direct pay.
  • Notes: These temp employees do not qualify for CalPERS membership.

  • What Benefits Ends:
    • First, confirm that FMLA is exhaused.
    • If unpaid FMLA, campus must generate a SCO Form 674 if the employee has a full pay period without pay requesting SCO cover the Health, Dental and Vision premiums for the period of non-pay status. Send form using Connect HR.
    • Once FMLA period is exhaused, then the Campus Benefits office  notifies CalPERS via My CalPERS and enters the perm event 710 code (description in CalPERS includes the wording 'Pending NDI.')
  • How Long Stay on SWB: NDI process can take 4-5 months
  • Notes: When EE returns to work, Campus Benefits office notifies CalPERS via My CalPERS and enters the perm event 712 code (description in CalPERS includes 'Chg to deduct-Return to Work.')
