About the CSU692R form
The CSU692R informs CalPERS of changes to retiree dental enrollment.
The form is sent only when a retiree dental enrollment or change is processed.
The form may be:
- Generated and sent automatically when you complete enrollment in the system, or
- Completed and submitted manually by you when you cannot complete enrollment in the system. See: Appendix: How to FAX or mail the CSU692R form
Example: for future-dated enrollments where no change is taking effect, you do not submit this form.
System entry or manual entry?
System entry refers to processing completed using CHRS pages.
Manual processing refers to processing completed outside automated system workflows and may involve CSU692R submission or CalPERS intervention.
Which method to use?
- If the termination exists in Job Data and the retirement and termination dates match, go to System entry: CSU Retiree Dental Enrollment page.
- Otherwise, process the enrollment manually and submit the CSU692R form manually to CalPERS.
Manual processing typically includes manual CSU692R submission.
System entry: CSU Retiree Dental Enrollment page
Step 1: Open the Navbar button.
Step 2: Navigate to Menu > Benefits > Enroll in Benefits > CSU Retiree Dental Enrollment
This page is only used for the initial Retiree Dental Enrollment (applies to the initial Retiree Dental Enrollment for staff and Faculty Early Retirement Program (FERP) participants). Any updates to the enrollment must be processed manually by contacting CalPERS.
Initial dental enrollments that were not entered in CHRS prior to retirement require manual intervention by CalPERS.
When enrollment is completed using this page, the CSU692R form is generated and sent automatically when required.
Step 3: Search for the employee:
- Enter search criteria.
- Select Include History.
- Click Search.
The CSU Retiree Dental enrollment page opens.
Step 4: Complete the CSU Retiree Dental page fields:
- Enter the Benefit Plan. Note that only FERP employees can be enrolled in Enhanced Dental coverage.
- Enter the Permitting Event Code. The value is 166 for the initial retiree dental enrollment.
- Enter the Permitting Event Date: the date on which the permitting event occurred (for example, the employee's retirement date).
- Enter the Received Date: the date that Benefits Office received the completed dental enrollment form.
- Enter the Zip Code for the employee if different from the current value. If the Zip code is not in the PSR Cross Reference Zip Code table, refer to the Address Out-Of-State ZIP Code Errors job aid.
Step 5: Use the following decision flow to decide whether to check the Exclude from PSR Interface check box, then act accordingly.
If you see the following message, enroll the employee into a basic dental plan: Only FERP employee may be enrolled in Enhanced Dental coverage.
Step 6: Click Save.
After you click Save, the fields are disabled. If you need to make a change, select the Correction Required checkbox to re-open the greyed-out data entry fields, make your changes, click on Save and then reprint the retiree dental form.
Step 7: Review the CSU PSR Status Update:
- Not Sent: This CSU PSR Status means that the record has not yet been sent to CalPERS. This is the transaction status prior to running the CSU PSR Outbound Interface process.
- Submitted: This CSU PSR Status is the transaction status after the CSU PSR interface has run and extracted the record onto the CSU PSR Outbound Interface process.
- Processed: This is the CSU PSR Status after successful processing by CalPERS.
- Error: This is the CSU PSR Status after a CSU or CalPERS processing error was encountered.
- Manual: This CSU PSR Status denotes that the transaction was in error status however the Ignore Error or Error Resolved checkbox was manually selected to resolve the issue on the CSU PSR Errors page.
Step 8: Click the Print Dental Form button to generate the Retiree Dental Form.
- The enrollment form opens in a separate tab.
Step 9: Close this tab when you are finished reviewing the form.
Enroll Dependents into Retiree Dental Plan
The CSU Retiree Dental Dependent Enrollment page is also used to enter retiree dental enrollments for dependents. A Retiree Dental Enrollment entry for an employee is required at the same time as the employee in order to initiate a Retiree Dental Enrollment for a dependent.
For this procedure, you start from the CSU Retiree Dental enrollment page already open with the employee that you just enrolled.
IMPORTANT: You cannot enter a new dependent on this page; you can select an existing dependent. Retirees need to contact their retirement plan to add new dependents after retirement.
Step 1: Enter the dependent's information in the Dependent Information area.
- Use the lookup to find the dependent.
- Enter the Zip code. The country field automatically updates with the Zip code.
- Optional: Click the Plus button to add another row.
Step 2: Optional: If you need to make any corrections, check Correction Required check box and then update the newly editable fields.
Step 3: Click Save.
Step 4: Click the Print Dental Form button to generate the Retiree Dental Form.
After you click Save, the fields are disabled. If you need to make a change, select the Correction Required checkbox to re-open the greyed out data entry fields, make your changes, click on Save and then reprint the retiree dental form.
