  • Contact Name, Relationship to Employee, Address Type, Contact Address, Contact Phone. 
  • Checkboxes: Primary Contact, Same Address as Employee, Same Phone as Employee.
  • Available for update in Self-Service.
  • Emergency contact information is recommended but not required for CHRS and is no longer a systemwide audit item.
