Update Your Final Paycheck Designee
Overview
This job aid shows you how to update your Final Paycheck Designee.
Background
Your Final Paycheck Designee is the person who receives your final paycheck if you die while employed at the CSU.
If you cannot access Employee Self Service, contact your HR administrator to update this information for you.
Change your Final Paycheck Designee
Step 1: On the Employee Self Service Homepage, click the CSU Personal Details tile.,
If you are on a different homepage, click the Homepage menu to select the Employee Self Service homepage.
Step 2: Select CSU Final Paycheck Designee from the Navigation panel.
Step 3: Read the information on this page.
- The page defines the terms of the designation.
Step 4: Optional: Decline Paycheck Designation.
- If you select Yes, then no one will receive your final paycheck. You do not need to enter the designee information fields.
Step 5: Click Instructions to view the instructions for this form.
- Close the Instructions window when you finish reading the instructions.
Step 6: Select the Designee Type.
- This field can be Person or Other. If you select Person, you must specify a Relationship.
Step 7: Enter the person’s name.
If you selected Designee Type Other, enter the Entity’s name.
Step 8: Select the Relationship of the Person.
- The recipient must be over 18 years old.
Step 9: Use the Lookup field to find the designee’s country.
Step 10: Enter the address of the designee.
Step 11: Enter the Phone number and Country code for this designee.
- Phone number is a free text field. Parentheses, dashes, or spaces are optional.
- Country code is not required for US phone numbers
Step 12: Optional: Click to expand up to three contingent designees to enter their information.
If the first designee dies before you, the first contingent designee on the list will receive your final paycheck.
Step 13: Read the paragraph, then toggle the signature slider to Yes if you agree to the statement.
Step 14: Click Save.
Your final paycheck designee is saved to the system.
